REACTIVATION BY EXAM (lapsed MORE THAN 24 months) Application

Please read these directions completely before continuing to complete this form online. Your request for reinstatement cannot be processed if it is incomplete, illegible, or includes false statements. 

You are eligible to reinstate your certificate via retesting if: 
  • Your New Mexico certification has lapsed more than 24 months, but you have been working in a nursing-related field for at least 24 months.  
  • You must complete your testing within six months of your eligibility to test.
NOTE: If your certification lapsed more than 24 months and you have not worked in a nursing-related field in the past 24 months, you must successfully complete a New Mexico state-approved nurse aide training program and then take and pass the Competency Examination to become certified.

INSTRUCTIONS: 
  • Complete and Submit this Reinstatement Waiver Application. You must upload the following document:
    • A letter from your employer on company letterhead listing the following information:
      • Employee's Name
      • Employee's Title
      • Employee's Hire Date(s)
      • A detailed list of employee's job duties
  • Once approved, you will receive an email with your Username and Temporary Password to sign in to your TMU© account.
  • Please refer to the New Mexico Nurse Aide Candidate Handbook  for instructions on Completing your Account and other important testing policies and updates.
 Once you have completed all the fields in this application, select 'Send Application' to submit it.
