Please complete this form if you are an Instructor in a New Mexico Health Care Authority (NMHCA)- approved Nurse Aide Training Program in New Mexico and would like to access TMU©. TMU© is the testing software platform through which the nurse aide competency evaluation will be conducted with D&SDT-HEADMASTER.
You can then enter new students' training records into the TMU© software and complete their training when they have successfully completed your program. Once a candidate's training record is completed, you can register them for their state competency evaluation testing.
NOTE: If you are an instructor for multiple training programs, please list each program name and program approval number under the Nurse Aide Training Program Affiliation(s) section. We will verify that you are a primary instructor with the training program coordinator(s). Please provide the training program coordinator's name and email address. If you are an instructor for multiple programs, please provide the training program coordinator's name(s) and email(s) with the corresponding programs.
